Meet our Team
Samantha S. Young, LNFA, Owner/Administrator
Samantha S. Young is an innovative entrepreneur with over 20 years of hands on experience, a degree in Health Professions from Texas State and is a licensed Nursing Facility Administrator. After ten years of working in a facility and running a successful non-profit program, Samantha decided that helping older adults remain independent in their own home and community was her passion in life and with that vision, opened her own home care business.
Samantha has operated, Practical Care Continuum, a successful non-medical home care agency in Austin since 2000. Her knowledge of the industry, business savvy, personal connections and willingness to share with others has proven to be a solid foundation for building her business.
Samantha is an active member of the American Society on Aging and the National Family Caregivers Association. As a company, Practical Care Continuum is a member of the Texas Association of Home Care and the Austin Woman's Chamber of Commerce.
As a caregiver herself Samantha understands the challenges and rewards of caring for a parent and juggling personal and professional responsibilities.

Elke Gasch, LMSW, Care Manager and Assistant Administrator Austin
Elke is a Licensed Master Social Worker who has a heartfelt commitment to helping older adults and their family members. Her love of helping seniors is inherited from her mother who sees many elderly clients in her beauty salon in Wisconsin. When she was young, Elke would spend Saturdays at the salon and would sit and talk with the “ladies” while they had their hair fixed.
Elke has over 10 years of experience working with seniors in a variety of settings. These include inpatient and outpatient geriatric mental health settings, a day program specifically for people with Alzheimer’s disease and related dementia, a nursing home and as a geriatric care manager. She joined Practical Care in April 2007. She is currently the Speakers Chair for the Austin Area Medical Social Work Committee, a member of the Cognitive Support Coalition, and active in many other professional health care organizations. Her professional experiences have enriched her understanding of the complexities of aging and the importance of looking at each client individually to determine their unique needs and what will best meet their quality of life. Elke is responsible for client assessments in central and South Austin. Elke is also available to provide educational in-services to professional and community groups.
